Tablet (NRN: 04-0898): Homatropine methylbromide USP 10 mg.

Pack size: Blister pack of 20’s, 100’s and Hospital pack of 500’s.

Syrup (NRN: 04-2160): Homatropine methylbromide USP 5 mg per 5 mL.

Pack size: 60 mL bottle.

Oral Drops (NRN:04-0210): Homatropine methylbromide USP 2 mg per mL.

Pack size: 15 mL bottle with dropper.

Pharmacology

Nospamin contains Homatropine methylbromide an antimuscarinic agent. It is a competitive inhibitor of the action of acetylcholine on the muscarinic receptor of autonomic effector sites innervated by parasympathetic nerves, as well as being an inhibitor to some extent of the action of acetylcholine on smooth muscles lacking cholinergic innervation.

It has a potent antispasmodic effect and minimal anti-cholinergic side effects.

Indications

Treatment of gastrointestinal spasms and as an adjunct in the treatment of peptic ulcer.

Contra-indications

In patients who have a history of Myasthenia gravis, closed or narrow angle glaucoma.

In patients who are sensitive to any ingredient in the formulation.

Precautions/Warnings

Treatment with Nospamin in acute myocardial infarction, thyrotoxicosis, cardiac insufficiency should be done with care. Not to be dispensed or repeated without medical prescription.

If symptoms persist for more than two days consult your doctor.

Interactions

Concomitant administration with other antimuscarinic drugs such as atropine, hyosine, antihistamines, butyrophenones, phenothiazine, and

tricylic antidepressants may cause potentiation of pharmacological responses.

Adverse Effects

These are typical of that observed after administration of other antimuscarinics. They include thirst, dilatation of pupils (mydriasis), with loss of accommodation (cycloplegia) on long term use.

Cardiovascular side effects such as bradycardia followed by relaxed tachycardia with palpitation, arrhythmias had been reported, following administration of toxic doses.

High doses often result in urinary urgency, difficulty and retention, constipation due to sudden drastic reduction in motility (paralytic lieus). Restrosternal pain may occur due to gastric reflux. Nospamin may cause dryness of the mouth and difficulty in swallowing.

Dosage & Administration

Adults:

One to two tablet (s) 3-4 times daily or as needed.

Children (1-3 years): ½ teaspoonful (2.5 mL) syrup 2-3 times daily; 3-6 years, One teaspoonful (5 mL) syrup 2-3 times daily; 6-12 years, One

tablet 2-3 times daily. Or as needed.

Infants:

Doses according to body weight: One Drop per kg body weight 3-4 times daily.

Doses according to age: Up to 3 months, 2-4 drops 3-4 times daily; 3-6 months, 4-6 drops 3-4 times daily; 6-12 months, 6-9 drops 3-4 times daily. Or as needed.

Infant doses are to be taken 10-15 minutes before feeding.

Storage/Handling Recommendations

Store in air tight containers. Protect from light.

Keep away from the reach of children.
